Diabetes Clinic
The medical management of diabetes is very
important to patients, their family members, friends, and to their
health care professional. That is one reason why the Outpatient
Diabetes Clinic was formed at Ivinson Memorial Hospital.
We know the medical management of this disease
can seem overwhelming, so we are here to help! Along with your
primary care physician, IMH has a "team" of health care
professionals who are available to help you manage this disease,
answer specific questions and provide you with information. You will
receive individualized attention and care from the diabetes
management team at IMH.
How the Diabetes Clinic Works:
- Before attending the clinic:
- Your primary physician must refer you to the diabetes
clinic.
- During the clinic:
- On the day of your scheduled diabetes clinic, you must check
in through outpatient admitting at IMH.
- Per your physician's discretion, blood may also be drawn
during the clinic (at an additional cost).
- The day of the clinic, you will see the following health
care professionals:
- physician
- admitting clerk
- nurse educator
- dietitian
- social work
- physical therapist
- After reviewing your needs assessment and receiving your
complete lab results, each member of the diabetes health care
team, as well as your personal physician, will visit with you
one-on-one. Your health care goals will then be established.
- After seeing each of the health care team members, you will
receive an individualized management plan and be rescheduled for
a follow-up visit in three to six months.
- You will then be discharged and are encouraged to work on
your personal health care goals set in conjunction with the
health care team members. During the next clinic visit, your
progress and any problems will be reviewed.
