This outpa­tient based health service is solely dedi­cated for indi­vid­u­als who have expe­ri­enced a cardiac event or other heart-related prob­lem. This physi­cian referred program is offered in two phases, Phase II and Phase III

phase II

This phase of the program is designed for an indi­vid­ual to begin after a recent cardiac event who has been discharged from the hospi­tal. The empha­sis in the Phase II program is on lifestyle assess­ment, behav­ioral changes, and exer­cise. The Phase II program is held three times a week for twelve weeks, depend­ing on your phys­i­cal condition. 

During this phase we provide:

  • One-on-one eval­u­a­tion and indi­vid­ual review of heart disease and risk factors.
  • Estab­lish personal goals for changes in lifestyle and assis­tance in moni­tor­ing progress and support.
  • Indi­vid­u­al­ized exer­cise and educa­tional programs.
  • EKG moni­tor­ing by a trained specialist.
  • Moni­tor­ing of heart rate and blood pres­sure checks before, during, and after treat­ment sessions by a trained specialist.

phase III

Phase III of the program is a contin­u­a­tion of Phase II which empha­sizes on the preven­tion of further cardiac prob­lems, and how to main­tain a heart healthy lifestyle, this is a self-pay program. 

During this phase you can expect:

  • Estab­lish personal goals for main­tain­ing changes in lifestyle and assis­tance in moni­tor­ing progress.
  • Empha­size on preven­tion of further heart complications.
  • Indi­vid­u­al­ized exer­cise and educa­tional programs.
  • Offer a support­ive group environment.
  • Moni­tor heart rate and blood pres­sure checks by a trained specialist.
