This out­pa­tient based health ser­vice is sole­ly ded­i­cat­ed for indi­vid­u­als who have expe­ri­enced a car­diac event or oth­er heart-relat­ed prob­lem. This physi­cian referred pro­gram is offered in two phas­es, Phase II and Phase III

phase II

This phase of the pro­gram is designed for an indi­vid­ual to begin after a recent car­diac event who has been dis­charged from the hos­pi­tal. The empha­sis in the Phase II pro­gram is on lifestyle assess­ment, behav­ioral changes, and exer­cise. The Phase II pro­gram is held three times a week for twelve weeks, depend­ing on your phys­i­cal condition. 

Dur­ing this phase we provide:

  • One-on-one eval­u­a­tion and indi­vid­ual review of heart dis­ease and risk factors.
  • Estab­lish per­son­al goals for changes in lifestyle and assis­tance in mon­i­tor­ing progress and support.
  • Indi­vid­u­al­ized exer­cise and edu­ca­tion­al programs.
  • EKG mon­i­tor­ing by a trained specialist.
  • Mon­i­tor­ing of heart rate and blood pres­sure checks before, dur­ing, and after treat­ment ses­sions by a trained specialist.

phase III

Phase III of the pro­gram is a con­tin­u­a­tion of Phase II which empha­sizes on the pre­ven­tion of fur­ther car­diac prob­lems, and how to main­tain a heart healthy lifestyle, this is a self-pay program. 

Dur­ing this phase you can expect:

  • Estab­lish per­son­al goals for main­tain­ing changes in lifestyle and assis­tance in mon­i­tor­ing progress.
  • Empha­size on pre­ven­tion of fur­ther heart complications.
  • Indi­vid­u­al­ized exer­cise and edu­ca­tion­al programs.
  • Offer a sup­port­ive group environment.
  • Mon­i­tor heart rate and blood pres­sure checks by a trained specialist.
